APPLICATION PROCESS

In order to receive services from PZHS, you must submit an application. Applications MUST be updated yearly to determine eligibility. The following documents are required:

• Household Income

  • Check Stubs

  • Social Security Award Letter

  • Unemployment Letter

• College or Vocational Institute

  • Letter from Institute

  • Class Schedule

• Child Care Expenses

  • Letter of Verification from Child Care Provider

• Disability or Handicap

  • Letter of Verification from Physician’s Office

• Military or Veteran

  • Documentation of Active Services

  • Discharge Papers
